Your information is not disclosed to any additional party without your consent unless required to do so by law or as needed for billing purposes. For patients 15 years and older, the patient needs to sign a release of information in order for us to disclose information to legal guardians.

If your child is requesting an appointment, you or they may make the appointment by scheduling online or calling our office. We apologize that it is frustrating to not be provided information on your child, but if your child is 15 years or older and has not signed a release of information (ROI), we will not disclose information to you. i lost my controlled medication. what do i do?
If you have lost your medication, you should file a police report and inform us, as well as the pharmacy, that you have lost your medication. Controlled medications will not be filled earlier than their fill date. To prevent loss of your medications, you should keep them in a secured location, out of reach of children, and should never travel with more medication than you need for your trip duration.
what should i bring to my first appointment?
You must bring a valid ID, and insurance card to the appointment.. Be prepared to pay for private pay or any copays or co insurance.
It is helpful to bring a list of previous and current medications to your first appointment.
WHAT IS YOUR CANCELLATION POLICY?
We require that you provide 24-hours notice should you need to cancel or re-schedule an appointment. We understand that situations out of our control arise, but excessive (up to 3) episodes of late cancellations or missed appointments (no show) may lead to termination of care. If you are more than 10 minutes late to an appointment, your appointment will need to be rescheduled. It is imperative you arrive on time and consistently attend your appointments to help maximize your mental health outcomes.
How do I get my medication refilled?
You are provided enough medication to get from appointment to appointment. If appropriate, refills will be ordered on the medication. It is your responsibility to ensure that you have the medication you need to avoid a lapse of care. If you miss an appointment, it is your responsibility to re-schedule prior to running out of medication. If you contact the clinic to request a refill, please allow 72-hours for the medication to be refilled. Please be aware that a prior authorization may delay getting medications and we do not have control to how long an insurance takes for prior authorizations.
